Short-Term Results of Trnasurethral Hyperthermia in BPH


Abstract
BPH is one of the most common pathology to afflict the aging male. Currently TURP is the gold standard treatment, however a more minimally invasive therapy would have considerable advantages, were it shown to be effective.
Transurethral hyperthermia(TUHT) is a new non-operative treatment for BPH. 103 patients with bladder obstruction due to BPH underwent TUHT with THERMEX-II, pre-treatment mean Boyarsky symptom scores were 2.21 in hesitency, 2.15 in intermittency,
2.10 in
residual sensation, 1.91 in nocturia, 1.50 in dysuria and the mean maximal flow rate was 9.86 ml/sec, mean weight of prostate was 30.4 grams. During treatment all patients felt considerable urgency but no patient to be discontinued therapy. At 3
months,
Boyarsky symptom scores had fallen to 0.42 in hesitency(P<0.001), 0.36 in intermittency(P<0.001), 0.36 in residual sensation(P<0.01). 0.74 in nocturia(P<0.001), 0.33 in dysuria(P<0.001), mean maximal flow rate increased to 16.32ml/sec(P<0.05),
and
the
weight of prostate was fallen to 26.02 grams(p>0.05). Overall 88% of patients were shown marked improvement symptomatically, but minimal in objective response. There were some minor complications including hematuria, transient urinary retention,
acute
epididymitis and so on. We concluded that transurethral hyperthermia(TUHT) has good efficacy, especilly in terms of bladder irritation symptoms, but flow rate improvement and reducing the weight of prostate has moderate to minimal. Further
studies
are
required to confirm these results.
